Ridesharing Services for Participants
OneWell Health Care offers reliable transportation through Uber Health, providing participants with safe and convenient rides. Covered under the Person/Family Directed Supports (P/FDS), Community Living, and Consolidated Waivers, as well as Base funding, this service empowers participants to access employment, services, and activities outlined in their Individual Support Plans (ISP).
Eligibility Requirements
To qualify for Uber Health, participants must meet these criteria:
Cost-Effective Option
1. Alternatives like bus passes, taxis, and county transportation must be ruled out.
2. Cost comparison with Medicaid Waiver trip costs is completed.
Practical & Safe Usage
Complete an intake with the Uber Health program specialist, including a safety screening:
• Can the participant identify the vehicle and driver?
• Are they able to communicate with the driver?
• Do they have a smartphone to use the Uber app?

How Uber Health Works
Intake Process:
1. Address screening and cost estimates are reviewed.
2. Costs are submitted to the supports coordinator for ISP approval.
Account Setup:
1. Participants (or caregivers) receive an invitation to join OneWell’s Uber Health account.
2. OneWell’s payment method is added to the Uber app, and rides are ready to begin.
Where Uber Health Can be Used
- Employment, volunteering, or social activities specified in the ISP.
- Participants under 21 can use it only for waiver-funded programs or competitive employment.
- Residential model participants can use it for transportation to/from integrated employment.
- Medical Appointments: Must explore Medical Assistance Transportation Program (MATP) first.
