Through our partnership with the San Diego Regional Center, OneWell Health Care proudly serves individuals in San Diego and Imperial Counties. We offer a wide range of specialized services designed to support independence including:
Supported Living Services (SLS)
Providing personalized support to individuals in their homes and communities, promoting independence while assisting with daily living tasks and personal goals.
Independent Living Services (ILS)
Empowering individuals to live independently by building essential skills and connecting them with necessary resources and supports.
Independent Living Transition (ILT)
Guiding individuals through the process of transitioning from supportive environments to independent living by fostering essential life skills, connecting them with community resources, and encouraging self-reliance.
Coordinated Family Support (CFS)
CFS providers collaborate with individuals and their families to create a personalized service plan. This plan focuses on identifying and addressing barriers to remaining in the family home, tackling specific challenges, and setting achievable goals. Services are delivered in a manner that respects the language and cultural background of each family, ensuring they are tailored to individual needs and adaptable to changing circumstances.
Eligibility Criteria:
- Adults aged 18 or older.
- Residing in the family home.
- Receiving services from a regional center.
Personal Assistance
Delivering individualized support to help individuals manage daily tasks such as mobility, personal hygiene, and meal preparation, enabling greater independence and an improved quality of life.
Respite Care
Offering temporary care in a secure and nurturing environment, ensuring individuals’ daily needs are met while allowing for a safe and comfortable experience.
Applied Behavior Analysis (ABA) Through Tricare
Providing evidence-based interventions to support individuals, particularly children with autism, by enhancing communication, social interaction, and behavior management through Tricare-funded services.

The Blue Envelope Program
