OneWell was founded as the IDD division of PAHM (PA Health Management) in 2020.

OUR HISTORY

In December 2012, PA Health Management (PAHM) was born. We created PAHM because we saw a need to create positive health outcomes for individuals in our community. In the beginning, we focused on providing Supports Coordination for individuals by being a Managed Care Organizations (MCOs) chosen by the Pennsylvania Department of Human Services to deliver Managed Long-Term Services and Supports (MLTSS). By spending time with each person one-on-one, building supportive teams, and working to understand the details of each unique case, we quickly began to grow. In 2020, we expanded our services to include Addiction Treatment Programs and Intellectual/Developmental Disability (IDD) Services. Through innovative ideas, we promoted a higher quality of life for over 5,000 individuals with physical & developmental disabilities, addiction, and the aging population.

Seeing a need for a new mission and vision for those in the IDD community, PAHM made the decision to separate Supports Coordination and IDD Services into separate entities in 2021. Starting in Pennsylvania OneWell has since expanded into both California and New Jersey to provide exceptional services in their communities.

We believe in establishing a personalized plan and including support preferred by individuals and their families to help reach personal and developmental goals while setting an overall goal to increase self-sufficiency, improve quality of life, and reduce stress.
