These services are designed to assist individuals with independent living, successful tenancy, and long-term housing stability while promoting choice, community inclusion, and self-sufficiency.
Supports may include assistance with finding and securing housing, completing housing applications, understanding and complying with lease agreements, communicating with landlords, coordinating with community resources, developing independent living skills, creating household routines, budgeting, and addressing issues that may place housing at risk. Services may also include ongoing support to help individuals safely and successfully remain in their homes over time.

1. Referral or Request
Services begin with a direct request from an individual, family member, guardian, case manager, or referring agency. Our intake team gathers initial information to understand housing needs, eligibility, and goals.
2. Benefits & Docs
We verify funding and collect required documentation, including proof of eligibility and service orders. Individuals are informed of coverage details before services begin.
3. Meet and Greet
An in-person meeting introduces individuals to OneWell to discuss housing goals and preferences. This helps determine fit and builds rapport. Required intake paperwork can also be completed during this visit.
4. Assessment
A qualified specialist completes an assessment reviewing housing history, goals, barriers, and independent living skills. This identifies priority supports for housing access and stability.
5. Plan Development
We create an individualized plan outlining action steps, skill-building strategies, and communication guidelines. Individuals review and contribute to this plan before supports begin.
6. Direct Services
Supports begin in the home or community. Services include housing search assistance, education on tenant rights, budgeting support, and early intervention to maintain housing stability.
7. Ongoing Support
We provide ongoing supervision to ensure supports align with goals. Progress is reviewed regularly, and plans are adjusted as needed. Transition planning occurs when stability is established.
