Services begin with a clinical referral from a hospital discharge planner, primary physician, or case manager. Our nursing coordinators perform an early medical review to screen for complex care eligibility.

Our dedicated authorization department verifies insurance configurations across Medicaid, Managed Care Organizations (MCOs), or special waivers to outline coverage guidelines.

3. In-Home Clinical Assessment

An RN Nursing Supervisor conducts an in-depth home visit to analyze medical status, high-tech nursing demands (ventilator, tracheostomy, feeding tubes), and environmental safety elements.

4. Physician Collaboration & POC

We combine forces with the patient’s prescribing physician to establish the formal Plan of Care (Form 485), detailing skilled nursing targets, medication protocols, and backup plans.
