1. Client Referred
Clients are referred to OneWell via a Service Coordinator or by reaching out to us directly.
2. Intake Scheduled
The Transportation team schedules a meeting to discuss individual goals and service compatibility.
3. Intake Progress
Our team begins the intake process and coordinates necessary authorization documentation.
4. Authorization Sent
A formal request is sent to the Service Coordinator for final approval to begin services.
5. Approval / Denial
Once waiver funding is approved, services move to activation. Private pay options are available if denied.
6. Activation
Onboarding includes app access and training to ensure you are ready for your first ride.
