Private Duty Nursing Qualification and Assessment

Private Duty Nursing at OneWell Health Care is available to participants through three ODP waivers: Person/Family Directed Supports (P/FDS), Community Living Waiver, and Consolidated Waiver. This vital service, comprised of Registered Nurses (RN) and Licensed Practical Nurses (LPN), is specifically designed for individuals aged 21 and older with Intellectual and Developmental Disabilities (IDD). Qualification is based on a careful assessment of specific criteria:

Medical Necessity

A critical consideration is whether the participant's clinical treatment requires nurse care or if a Direct Support Professional (DSP) with nurse supervision can provide the necessary support.

Respiratory Stability

Participants must be assessed to determine if they have an unstable airway condition that could lead to respiratory arrest without immediate nursing intervention.

Life-Supporting Equipment

Nursing support may be needed to maintain crucial life-supporting equipment within the participant's home.

In the domain of Private Duty Nursing at OneWell Health Care, thorough qualification and assessment are vital. This process, often conducted through an evaluation, establishes the need for nursing services and designates whether a Licensed Practical Nurse (LPN) or a Registered Nurse (RN) should provide the care. Participants can qualify for LPN, RN, or a combination of both levels of care, extending to 24 hours a day, seven days a week when the evaluation supports such requirements.

Nursing Care Plan and ISP Documentation

The qualification process doesn’t end with assessment alone. A nursing care plan from the participant’s designated nursing provider is essential to validate the continued need for this level of care and its appropriateness in addressing their unique and evolving requirements. The nursing level of care must also be documented as an outcome within their individual support plan (ISP).

Service Availability for Adults Aged 21 and Over

Private Duty Nursing is an exclusive service for adults aged 21 and over. For children under 21 who require medically necessary shift nursing, coverage typically falls under private insurance, Medical Assistance (MA), or a combination of both. This coverage is provided through the Early Periodic Screening Diagnostic Treatment (EPSDT) benefit. As participants approach their 21st birthday, it becomes crucial to perform a new nursing assessment to evaluate their current nursing needs since they no longer automatically receive nursing through the Office of Developmental Programs (ODP).

Payer of Last Resort: Documentation Requirements

As part of the Office of Developmental Programs (ODP) policy of being the payer of last resort, participants are obligated to provide documentation proving that their private insurance, Medicare, or MA does not cover shift nursing, or that they have reached the benefit limit for the nursing services they can receive. This documentation may include a copy of the policy or a written statement from the insurance company, clearly indicating that the requested nursing service exceeds coverage limits or is not included in the policy.

Seizure Activity Tracking

Participants who experience seizures receive specialized care, with nursing staff trained to monitor and respond to seizure activities.

Medication Administration

Administering medications and managing continuous feeds are integral parts of our nursing services to ensure proper health maintenance.

Personal Care

Personal care, including changing briefs, providing bed baths, and assisting with dressing, is conducted with the utmost care and sensitivity.
