The OneWell Personal Care Process
Our Personal Care program follows a clear and supportive process so individuals and families know what to expect from the initial inquiry through ongoing services.
1. Referral or Request
Services may begin with a direct inquiry from an individual or family, or through a referral from a provider. Our intake team gathers initial information to understand care needs, eligibility, and service preferences.
2. Benefits & Docs
We verify funding sources and collect required documentation, such as proof of eligibility and physician’s orders. Families are guided through OneWell’s intake forms and informed of coverage details.
3. Meet and Greet
An in-person meeting introduces families to OneWell to discuss routines and preferences. This allows for relationship-building and thoughtful matching with the most appropriate Personal Care staff.
4. Initial Assessment
A qualified supervisor completes an assessment involving interviews and observation. This identifies strengths and priority supports related to personal care, mobility, and daily routines.
5. Care Plan Development
Based on assessment findings, we develop an individualized plan outlining specific supports, safety considerations, and preferences. Families are invited to review the plan before services begin.
6. Direct Services
Care is provided by trained staff in the home or community. Support includes hygiene, transfers, meal preparation, and light housekeeping. Service hours vary based on assessed need.
7. Ongoing Oversight
Supervision ensures services remain safe and consistent. Plans are reviewed regularly. Transitions occur when care needs change, with collaborative planning based on the person’s progress.
