Participant Assistance & Care (PAC)
Participant Assistance and Care (PAC) services help individuals with intellectual and developmental disabilities live safely and successfully in their own homes, stay engaged in their communities, and reduce the need for institutional care. Services include support with activities of daily living, personal care, mobility, and other essential tasks through hands-on assistance, prompting, reminders, supervision, and monitoring to promote the individual's health, safety, and independence.
A family member may serve as the PAC provider if it is determined through the person-centered planning process that they are the most appropriate choice. This decision is documented in the Person-Centered Individual Support Plan (PCISP) and is reviewed annually by the Individual Support Team (IST) to ensure the arrangement continues to meet the individual's needs.
