What determines whether a family member can be hired
Family caregiver rules are set by each state, waiver, and payer — not by OneWell. Three things usually decide the answer.
The individual's funding
Which waiver or funding source authorizes their services, and whether that program permits payment to a relative.
Your relationship
Several states restrict payment to legally responsible individuals — such as a spouse or the parent of a minor child — while allowing other relatives.
The authorized service
Respite, personal care, and community supports are often open to family caregivers. Nursing and behavioral services usually require licensure or certification.

California
Supporting individuals and families through California Regional CentersOneWell partners with California Regional Centers to deliver individualized, person-centered services for individuals with IDD. Under California Title 17 requirements, parents are considered natural supports and cannot be hired by OneWell as paid caregivers.
Eligibility Requirements
- For authorized respite services only — an eligible family member (other than a parent) may be considered
- The family member must not live in the same household as the individual receiving services
- All hiring must be authorized by the Regional Center
- Must meet California regulations and OneWell's employment requirements
Services available to family caregivers
Important: Family caregiver hiring in California is limited to respite services only. Parents may not be hired as paid caregivers under California Title 17. All arrangements require Regional Center authorization.

Pennsylvania
Helping Pennsylvania families stay togetherOneWell works with the Pennsylvania Office of Developmental Programs (ODP) to support eligible families through person-centered services.
Eligibility Requirements
- The individual must be authorized for the service in their ISP
- The family member must meet all ODP and agency employment requirements, including background checks and required training
- The service must permit delivery by a relative under the applicable waiver service definition
- The arrangement must not violate conflict-of-interest, licensing, or other waiver requirements
- Legally responsible relatives (e.g., parents of minor children or spouses) may face restrictions on certain services that replace duties they are already legally obligated to provide
Services available
The 40/60 Rule (Pennsylvania ODP): A single relative may provide a maximum of 40 hours/week of IHCS and/or Companion services. Multiple relatives collectively may provide up to 60 hours/week. Provider agencies are responsible for monitoring compliance.

Connecticut
Family-centered support through Connecticut DDSOneWell partners with the Connecticut Department of Developmental Services (DDS) to help individuals receive quality supports in their homes and communities.
Eligibility Requirements
- The individual must be eligible for DDS services and enrolled in the applicable Medicaid waiver
- The service must be authorized through the individual's Person-Centered Plan (PCP) or Individual Plan
- The family member must meet all provider employment requirements, including background screening, training, and service-specific qualifications
- The provider agency must be a DDS-qualified provider for the applicable service

Georgia
Supporting Georgia families every step of the wayOneWell provides services through Georgia Medicaid waiver programs. Family caregiver hires under NOW/COMP waivers are permitted only under Participant Direction and require DBHDD approval.
Eligibility Requirements
- Approval is based on documented extenuating circumstances
- A Family Hire Request Form must be submitted through the Support Coordinator
- At least two qualifying extenuating circumstances are required (e.g., lack of qualified workers in the area, specialized skills, or cost-effectiveness)
- Family caregivers must meet all provider qualifications, training, documentation, and service requirements
- Under GAPP, parents and family members may provide Personal Care Services if employed by an enrolled GAPP provider and all Medicaid requirements are met

Indiana
Caring for Indiana familiesOneWell partners with families to provide individualized services in Indiana. Family caregiver eligibility is governed by BDS/DDRS requirements.
Eligibility Requirements
- Parents, stepparents, and legal guardians of minor waiver participants may NOT receive payment for waiver-funded services
- Other relatives (excluding spouses) may provide services if employed by an approved BDS/DDRS provider
- Family caregivers must be at least 18 years old and meet all provider qualifications
- Must be selected through the Person-Centered Individualized Support Plan (PCISP) with documented justification
- Paid relatives providing residential habilitation to adult participants are limited to 40 hours per week per caregiver

New Jersey
Supporting families across New JerseyOneWell helps eligible families navigate New Jersey Division of Developmental Disabilities (DDD) services while providing high-quality person-centered supports.
Eligibility Requirements
- The individual must be eligible for DDD services and enrolled in the Community Care Program (CCP) or Supports Program (SP)
- The individual must also be enrolled in New Jersey Medicaid (NJ FamilyCare)
- The service must be authorized in the individual's Service Plan through the Support Coordinator
- The family member must satisfy all provider employment requirements, including background screening, required training, and service-specific qualifications
- The provider agency must be an approved Medicaid/DDD provider for the authorized service

Personalized supports for Ohio familiesOneWell provides individualized home and community-based services in Ohio. Parents of minor children and spouses may serve as paid caregivers only under specific conditions.
Eligibility Requirements
- Parents of minor children and spouses may be hired only when no other willing and able provider is available AND DODD determines health and safety can be ensured
- Must meet Extraordinary Care requirements
- Generally limited to 40 hours per week unless an exception is approved
- Parents of minor children and spouses may NOT provide respite services
- Parents of adults, adult children, grandparents, siblings, and step-relatives may serve as paid caregivers subject to DODD and Medicaid requirements

South Carolina
Supporting families through DDSN and DHHS servicesOneWell partners with the South Carolina Department of Disabilities and Special Needs (DDSN) and SCDHHS to help eligible individuals remain safely at home while receiving quality supports.
Eligibility Requirements
- Spouse of the Medicaid participant (including legally separated spouses)
- Parent, stepparent, or foster parent of a minor participant
- Court-appointed guardian of an adult participant
- Any other legally responsible adult or legal guardian
- Primary caregivers are not eligible for reimbursement for respite services

DDSN Directive 735-02-DD: A Statement of Legal Responsibility must be completed prior to authorization. Family caregivers must meet the same hiring, training, competency, documentation, and provider standards as non-family caregivers.

Can I really be paid to care for my own family member?
In many cases, yes. It depends on your state, the individual's waiver or funding source, the specific service authorized, and your relationship to them. Tell us the details and we will confirm your eligibility before you commit to anything.
Can a parent be paid to care for their minor child?
Some programs allow it, others restrict payment to legally responsible individuals such as a parent of a minor or a spouse. The rule differs by state and by waiver, so we check the individual's specific authorization rather than giving a general answer.
Do I need experience or a certification?
Not for most services. OneWell provides the training your state requires. Nursing and behavioral services are the exception — those need the appropriate license or certification.
What do I have to complete before I can start?
Typically background checks and state clearances, required training, and standard employment paperwork. We tell you exactly what applies in your state and track every item with you.
How am I paid, and how are hours tracked?
You are a OneWell employee on a regular payroll cycle. Visits are recorded through Electronic Visit Verification, so hours are documented accurately and paid on schedule.
How long does it take to get started?
Our intake team responds within one hour. From there, the timeline depends on how quickly clearances and training are completed — and on your state's authorization process. We keep you updated at every step.
Is there any cost to my family?
No. Intake, eligibility review, and care coordination are provided at no cost to you.
