What Is Coordinated Family Support (CFS)?

Coordinated Family Support is a family-focused service model that provides assistance, planning, and coordination for individuals living with their families. Rather than replacing family care, CFS strengthens it—offering tools, training, and access to resources that help families better support their loved ones in everyday life.

Services may include guidance on care routines, connecting families to community resources, developing structured support plans, and helping navigate available programs and services. At OneWell, CFS is built around collaboration—working closely with families to create practical, effective support systems.
