What Is Physical Therapy (PT)?

Physical Therapy (PT) is a rehabilitative service focused on improving movement, strength, balance, mobility, and overall physical function. It is designed for +21 age adults experiencing mobility limitations, pain, physical impairments, developmental conditions, or functional challenges related to injury, illness, disability, or long-term health conditions. At OneWell Health Care, we proudly provide IDD Physical Therapy and support individuals with Intellectual and Developmental Disabilities (IDD), Autism Spectrum Disorder (ASD), neurological conditions, and physical disabilities through highly individualized, goal-focused treatment plans.

 

Services may include therapeutic exercises, mobility training, balance and coordination therapy, transfer training, gait training, postural support, pain management techniques, adaptive movement strategies, and caregiver education to support long-term physical wellness and independence. We specialize in Physical Therapy for Developmental Disabilities, helping individuals safely navigate daily routines and improve participation at home and in the community. Our team also provides Home-Based PT Services and In-Home Physical Therapy in Pennsylvania, making therapy more accessible and comfortable for individuals who benefit from receiving care in familiar environments.

OneWell Health Care provides Waiver-Funded Physical Therapy Services in Pennsylvania for individuals receiving support through the following programs. We work closely with families, Supports Coordinators, physicians, and waiver teams to develop personalized therapy plans that align with waiver goals, clinical recommendations, and long-term outcomes.

Person/Family Directed Support (P/FDS) Waiver

Designed for individuals with Intellectual and Developmental Disabilities (IDD) who live with family members or in community settings and need support services that promote independence and daily functioning.

Community Living Waiver

Provides flexible, community-based supports for individuals who require assistance with everyday living, mobility, safety, and participation in home and community environments.

Consolidated Waiver

Supports individuals with more significant medical, behavioral, or physical support needs who require comprehensive long-term services and individualized care planning.

Getting Started

The OneWell Physical Therapy Process

Our physical therapy program follows a clear, supportive pathway so families know what to expect from the very first contact through ongoing services.

1. Referral or Request

Services begin with outreach from family, supports coordinators, or providers via email or phone. Our coordinator will then contact the family to discuss the referral.

2. Document Submission

The program coordinator requests key documents including a copy of the ISP, completed referral forms, and copies of insurance cards.

3. Intake Meeting

An in-person or virtual meeting introduces families to OneWell, reviews the waiver process, discusses goals, and helps us match the individual with the right therapist.

4. Orders & Insurance

Our therapy team obtains a signed physician’s order and insurance denial information, then submits a critical revision request for an initial assessment.

5. Initial Assessment

A licensed therapist evaluates the individual in their home or community, creating a holistic treatment plan with input from the family and caregivers.

6. Second Critical Revision

Following the assessment, we obtain a new physician’s order with the recommended frequency and submit a revision for ongoing treatment units.
