Getting Started

The OneWell Physical Therapy Process

Our physical therapy program follows a clear, supportive pathway so families know what to expect from the very first contact through ongoing services.

1. Referral or Request

Services begin with outreach from family, supports coordinators, or providers via email or phone. Our coordinator will then contact the family to discuss the referral.

2. Document Submission

The program coordinator requests key documents including a copy of the ISP, completed referral forms, and copies of insurance cards.

3. Intake Meeting

An in-person or virtual meeting introduces families to OneWell, reviews the waiver process, discusses goals, and helps us match the individual with the right therapist.

4. Orders & Insurance

Our therapy team obtains a signed physician’s order and insurance denial information, then submits a critical revision request for an initial assessment.

5. Initial Assessment

A licensed therapist evaluates the individual in their home or community, creating a holistic treatment plan with input from the family and caregivers.

6. Second Critical Revision

Following the assessment, we obtain a new physician’s order with the recommended frequency and submit a revision for ongoing treatment units.

7. Ongoing Care

The therapist provides individualized treatment, adjusting as needed. We integrate family consultation to support progress.
