Home Modifications Process

➜   For modifications exceeding $1,000, three estimates are required. OneWell will provide one estimate, and the family must obtain the other two.

➜   To proceed, either a letter of medical necessity from the primary care physician or an Occupational Therapy assessment is required, along with the three estimates, when the Supports Coordinator submits the request to ODP for review.

➜   If OneWell is selected to complete the modification, we will supply all necessary materials and labor. After the project is completed, we will submit for reimbursement.

Bathroom: installation of adaptive bathtubs/showers, grab bars, fold down shower chairs, handheld showers, adapted fixtures, raised toilets, non-slip flooring.

Kitchen Modification: lower cabinets and countertops for wheelchair accessibility, installation of accessible sinks/fixtures, accessible storage, non-slip flooring

Widening Doorways: enhance accessibility throughout home

Sensory Modifications: adaptations for noise reduction, softened lighting, visual support, tactile adaptations, temperature regulation

Modifications for Behavioral Concerns: shatterproof windows, padding for walls

❌ Adaptations NOT covered under home modifications: Stair lifts, Elevators, Ramps, Durable Medical Equipment (DME) & General Maintenance ❌
