Our Home Modification Program at OneWell Health Care is committed to improving independence and quality of life for our participants by making their homes accessible. It must be warranted as a result their disability. We aim to optimize functionality of living spaces according to our participant’s abilities. This client-centered program is committed to enhancing safety, comfort and overall well-being.

Getting Started
Home Modifications Process
Three Estimates
For modifications exceeding $1,000, three estimates are required. OneWell will provide one estimate, and the family must obtain the other two.
Medical Necessity or OT Assessment
To proceed, either a letter of medical necessity from the primary care physician or an Occupational Therapy assessment is required, along with the three estimates, when the Supports Coordinator submits the request to ODP for review.
Materials, Labor, and Reimbursement
If OneWell is selected to complete the modification, we will supply all necessary materials and labor. After the project is completed, we will submit for reimbursement.

Bathroom Modifications
Installation of adaptive bathtubs/showers, grab bars, fold down shower chairs, handheld showers, adapted fixtures, raised toilets, and non-slip flooring.
Kitchen Modifications
Lowered cabinets and countertops for wheelchair accessibility, installation of accessible sinks/fixtures, practical storage solutions, and safer non-slip flooring.
Widening Doorways
Strategic expansion and structural structural modifications to door frames to smoothly enhance wheelchair and walking-aid accessibility throughout the entire home.
Sensory Modifications
Adaptations specifically targeted for noise reduction, softened calming lighting, vital visual support features, tactile adaptations, and precise temperature regulation adjustments.
Behavioral Modifications
Protective environmental adjustments including the integration of specialized shatterproof windows and protective padding for walls to optimize personal safety.
